I am somewhat new to PHP as well. I define these large areas of the web page in single blocks.

I have a header.php file which can take some arguments ($title, $navigationLinksArray etc.). This file contains the entire top block. It also calls my general functions page and style sheet. I do the same at the footer.

then I have my page.php include the header and footer at the top and bottom and then add content specific to the page in between.

This sounds similar to what you are doing, maybe a little different.

As far as speed I am not serving millions of pages but the header typcally is included in less than .007 seconds.
